• I authorize investigation of all statements in this application for employment as may be necessary in arrive at an employment decision.

• I freely grant consent for employers, schools, law enforcement agencies, other individuals and organizations to release any information to investigators, personnel staffing specialties and other authorized employees of the company, including criminal conviction history or credit history information.

• I hereby release the above agency from any and all damages or claims for damages arising either directly of indirectly as a result of this service.

• If hired, I understand that false or misleading information given in my application or interview may lead to discharge.

• I agree to submit to any pre-employment drug screening required by the employer, and for the results thereof to be released to the company.

• If hired, I agree to submit to any required drug/alcohol screening and for the results of said tests to be released to Wagner Paving, Inc.

• I understand that the information provided below will be used only for the purpose of the pre-employment background investigation and, if hired, any other testing as required (i.e. annual driving record for insurance purposes, specific project background investigations required, annual FMCSA Clearinghouse queries, etc.).

• If hired, I consent to an annual review of my driving record for as long as I am employed at Wagner Paving, Inc., to verify I am insurable to operate company owned vehicles.

• If hired, and ifl hold a CMV driver's license, I consent to an annual limited query through the FMCSA Commercial Driver's License Drug and Alcohol Clearinghouse to determine whether drug or alcohol violation information about me exist in the Clearinghouse for as long as I am employed at Wagner Paving, Inc.

• I understand that if hired this release of information and consent will remain in effect for the duration of my employment at Wagner Paving, Inc.

Paving Crew Member: Supports the paving operation, works with the crew, and gains experience in the field.

Concrete Crew Member: Supports the concrete operation, works with the crew, and gains experience in the field.

Driving & Logistics

CDL Driver: Operates dump trucks to haul asphalt and other materials.

Lowboy Driver: Transports heavy equipment to and from job sites. Requires a Class A CDL.

Maintenance & Shop

Heavy Equipment Mechanic: Performs routine maintenance and repairs on company vehicles and construction machinery to ensure safe and reliable operations.
